How long is my Combined Cares Card valid?
Your Combined Cares card once activated does not expire and can be used for as long as you like with no obligation to continue.
How often may I use my Combined Cares Card?
You can use your Combined Cares card as often as you like. There are no limits on usage.
How do I know if I've received my discount?
The fees of participating providers are available online to members. If you have questions about your charges, please contact us at (800) 290-0523 between 7AM to 7PM CST Monday – Friday.
How do I get a replacement Combined Cares Card?
Click the Print Card tab at the top of this page to print your replacement card.
May I use my membership when I travel away from home?
Your membership can be used at any participating provider in your area and throughout the US – a great bonus when you're traveling.
